Error installing OMSA failing on one of three identical hosts.

R620’s using for lab testing, all three running 6.5.0 Update 3 (Build 13932383) with identical processors, RAM, drives (10x900GB SAS).

Two machines OMSA installed, rebooted and I can now access. On the third machine I get the following error.

[InstallationError] Failed updating the bootloader: Execution of command
/usr/lib/vmware/bootloader-installer/install-bootloader failed: non-zero code returned

I found a few sites talking about disk space when the error says nothing along those lines. ESX boots from a 16GB USB drive and has 4TB locally to play with.

I think this might be it but stuck on what to do next. vSphere shows the USB disk capacity as NAN. The other two both show 16GB.
